Online Cancel Service Request Form

If you are moving out of your home and out of the JCW Service Area and you need to cancel service at your old service or current service location, please complete the Cancel Service Request form. (You'll need Adobe Acrobat Reader to open the form.)

Online Transfer Service Request Form

If you have moved to a new service location and it is in the JCW service area, and your previous location is serviced by JCW, and you need the wastewater service for the old service location taken out of your name, please use the Transfer Service form. (You'll need Adobe Acrobat Reader to open the form.)

Will be accepted Monday through Friday until 5:00 PM.

Current Hours

8:00 a.m. - 5:00 p.m. Monday through Friday.
